If your daughter is interested in playing soccer at Upper Arlington High School, please complete and submit the form below. She also must be registered through the school’s 8to18 website. Please visit the Athletic Department’s Forms & Information page to learn more about creating an 8to18 account and submitting a required physical form with the school before participating.
